Initial Assessment Appointment

The aim of your initial assessment appointment is to help us to understand the overall nature of your difficulties and how we may be of assistance to you. We will start by asking you some background questions about you and your current circumstances. If you have already completed our background information questionnaire we shall review this with you and ask some additional questions to add to the information you have already provided.

If you were referred to us for help with work issues we may ask if you could bring along details of your employment record (such as a CV, if you have one available), as this will save time in the initial assessment appointment.

We shall then review the nature and effects of your brain injury and what input you have received from other services including hospital care, any in-patient or out-patient rehabilitation or any home care support. We shall then focus on your current difficulties and needs, so that we can discuss with you how we may be of assistance to you.

For this initial appointment we usually ask that, if possible, that you are accompanied by a relative or close friend. This helps us to gain a broader understanding of the nature and effects of your brain injury so that we can consider how we might be of assistance.

Practical Information

Your initial assessment appointment at the Brookside Clinic Centre will usually take approximately two and a half hours.

Along with your appointment letter we usually you send a map indicating the location of the Brookside Clinic together with information on travel and parking.

If you are unable to get to the appointment please let us know as soon as possible as we may be able to book hospital transport for you.

If you are unable to attend on the date suggested, please let us know as soon as possible so that we can arrange a more convenient time for you. This would also enable us to offer the appointment to someone else.
